PAR - Q
For most people, physical activity should not pose any problem or hazard. PAR-Q has been designed to identify the small number of adults for whom physical activity might be inappropriate and those qho should have medical advice concerning the type of activity most suitable.
Physician Release Form
Many times you will need the client's doctor approval before you go ahead with training the client. Use this form to get recommendations and limitations from the client's physician.
